4-D ultrasound. The images provided by this high-tech
equipment are detailed beyond anything available up to
this time.
Dr. Banias and Genesis OB/GYN is one of the few physicians
and OB/GYN offices that own the latest GE 3-D/4-D
ultrasound machine (Voluson 730). He is currently one of
the only physicians who offer 3-D/4-D images to patients.
The following information is provided for patients who
wish to receive 3-D/4-D images.
• Dr. Banias offers this service to all expecting mothers
as long as they agree to pay for the expense of the exam.
• The recommended time for 3-D imaging is in between 28 to
32 weeks gestation. At this time the fetus has a
substantial amount of fat on its body and the images tend
to turn out at a higher quality than at an earlier
gestation. After the recommended time the images are more
difficult to obtain due to the decreased amount of
amniotic fluid. Dr. Banias' office will not attempt 3-D
images before the fetus is at least 18 weeks gestation.
• The patient must have previously received a documented,
diagnostic ultrasound before receiving a 3-D/4-D
ultrasound. The diagnostic ultrasound can be preformed by
Dr. Banias' staff with a written referral from the
patient's physician. The cost of the diagnostic ultrasound
will then be submitted to the patient's insurance company.
The cost of the 3-D ultrasound will still be the
responsibly of the patient.
• Dr. Banias' staff reserves the right to decide whether
or not the ultrasound pictures are satisfactory. It is up
to the baby's cooperation for the quality of the images.
If the images are unsatisfactory to the ultra-sonographer,
the ultra-sonographer will then decide whether or not to
reschedule for a follow-up appointment. There is no
guarantee of perfect images.
• The cost of a 3-D/4-D ultrasound is $250.00 cash (credit
cards will be accepted). This is to be collected before
the service is rendered. Due to the amount of time allowed
for the procedure this fee is non-refundable (whether or
not satisfactory images are obtained).
• The cost of the procedure includes numerous black and
white images, a few color images, a DVD of the
ultrasound, and a CD containing 3D images.
• Before the service is provided, the patient will be
asked to sign a release form stating that they understand
and agree to the above information.
